Please note that before a 6(d) certificate can be issued, the unit must have a zero balance through the closing date and we must have the information below completed and returned to our office. Please return this form along with a check payable to “RMC Management Corporation” for $100. If this application is submitted less than 5 days from the closing date, a rush fee will be applied and the fee will be $125 payable to RMC Management Corporation.
